Supports corporate objectives by professionally representing the RMHP Case Management Agency. Utilizes a general knowledge of administrative support, excellent customer service, and solid conflict management skills to provide an excellent virtual first impression of the health plan and corporation. Utilizes a demonstrated competency of Long Term Services and Supports to provide high level case aide support and potential back-up for case management.



If you are located in Fremont, Custer, Chaffee Counties, you will have the flexibility to work remotely* as you take on some tough challenges.



Primary Responsibilities: -



  • Serve as Case Aide and provide administrative and case management support to Case Managers/HSS Coordinators as needed.

    • Complete quarterly phone contacts
    • Complete provider and BHO monitoring contacts every 180 days
    • Complete verbal consent lists and obtain verbal consent from members as needed
    • Complete monthly summaries for BI caseloads
    • Complete monthly HCA phone contacts


    • Frequently handles difficult situations with individuals of varying emotional states. Responsible for diffusing the situation, intervening as appropriate, and ensuring clients and their families are cared for as much as possible. Utilize on-site personnel, security, or other resources as needed.
    • Provide first point of contact coverage virtually in a cordial and professional manner. Determine their needs and provide information and/or notify the appropriate individual.
    • Receive and route incoming calls as needed to ensure effective customer phone service. Respond to internal and external contacts in a patient, friendly and helpful manner, often working to diffuse tense situations.
    • Maintain accurate and updated resource materials.
    • Provide additional administrative duties such as preparing communications, printing labels, and faxing documents. Performs other administrative support duties as necessary.
    • Other functions may be assigned and management retains the right to add or change the duties at any time.

      Required Qualifications: -



      • Must meet one of the following:

        • Bachelor's degree in one of the human behavioral science fields such as human services nursing, social work, psychology, public health
        • 5+ years of relevant experience in the field of LTSS, which includes developmental disabilities
        • Combination of education and experience that would enable performance of the full scope of the position. -Relevant experience is defined as:

          • Experience in one of the following areas: long-term care services and supports, gerontology, physical rehabilitation, disability services, children with special health care needs, behavioral science, special education, public health or nonprofit administration, or health/medical services, including working directly with persons with physical, intellectual or developmental disabilities, mental illness, or other vulnerable populations as appropriate to the position being filled
          • Completed coursework and/or experience related to the type of administrative duties performed by case managers may qualify for up to two (2) years of required relevant experience




          • 1+ years of experience with MS Office, including Word, Excel, and Outlook -
          • Reliable transportation and the ability to travel up to 25% within assigned territory to meet with members and providers -
          • Reside in Fremont, Custer, Chaffee Counties or nearby surrounding counties

            Colorado Residents Only: The hourly range for Colorado residents is $16.00 to $31.44.
