Job Description:
HCA Healthcare seeks a dedicated RN Case Manager for our
Inpatient Rehabilitation Facility (IRF) to oversee patient care
management ensuring optimal recovery outcomes and superior care
coordination. This role involves working collaboratively with
multidisciplinary healthcare teams to plan, monitor, and evaluate
healthcare plans tailored to our patients' complex medical needs.
The ideal candidate will advocate for patients' needs, striving to
deliver seamless, holistic care aligned with HCA Healthcare's
commitment to improving human life. Assess patient condition and
needs to develop tailored care plans Coordinate with medical
professionals and support staff to implement effective patient care
strategies Monitor patient progress and modify care plans as
necessary Ensure compliance with healthcare regulations and
standards Educate patients and families about care processes and
post-discharge treatments Facilitate the efficient use of resources
within the facility Document patient care activities clearly and
accurately Strong clinical care knowledge and problem-solving
Effective communication and interpersonal skills Proficient in
medical software and documentation Exceptional organizational and
case management skills Awareness of healthcare laws and ethics
