Job Description:
Aspire Health Partners is seeking a dedicated and compassionate
Advanced Practice Registered Nurse (APRN) to join our
multidisciplinary healthcare team. This role is critical in
delivering high-quality care to patients dealing with mental health
issues, substance abuse, and challenges associated with
homelessness. The APRN will work closely with healthcare
professionals to provide comprehensive, individualized care aimed
at improving patient outcomes and enhancing the quality of life for
our community members. Conduct comprehensive patient assessments to
gather information about mental health, substance use, and physical
health issues. Develop, implement, and evaluate individualized care
plans based on patient assessments. Prescribe medications and
therapies, monitor efficacy, and make necessary adjustments.
Provide crisis intervention and urgent care as needed. Educate
patients and their families on treatment plans, health management,
and wellness strategies. Collaborate with other healthcare
professionals and service providers to coordinate care. Participate
in training and professional development activities. Ensure all
care delivery adheres to state guidelines and regulations.
Expertise in psychiatric and medical health assessment, diagnosis,
and treatment. Strong interpersonal and communication skills.
Ability to manage time effectively and make sound decisions in
emergency situations. Proficient in clinical decision-making and
care management. Knowledge of mental health laws and regulations.
Medical Insurance Dental Insurance Vision Insurance Valid and
active state Nurse Practitioner license. Certification in
psychiatric-mental health is preferred.
