Job Description:

The top 5% of the Medicare Advantage population is the highest risk and highest cost segment that consumes upwards of 50% of all utilization. This population presents a challenge in the coordination of medical, behavioral, substance abuse, and social determinants in improving their health outcomes. We are embarking on a new care program that aims to treat this population to optimize Patient health. The care program will provide a holistic approach to these Patients in both a virtual and home setting. This role will be functioning as an extension of the Primary Care Physician (Extensivist). The Extensivist will collaborate and supplement the PCP in the provision of care that will be rendered for our highest risk Patient population. The Extensivist Physician will work with a mutli-disciplinary team to create an environment for regular monitoring and interventions for the various conditions the Patient may have. Our mission is to treat Patients with Quality of Care virtually and/or in the comfort of the Patient's home.

Essential Functions:
Physician with demonstrated expertise in treating and managing Medicare Advantage populations with multiple comorbidities
Extensivist Physician will provide management and oversight of patients until conditions improve and stabilize
Virtual medical management (see, treat and co-manage) of high risk, frail population
Clinical oversight of, mentor, and assist Advanced Practice Registered Nurse (APRN), Psychiatric APRN, Registered Nurse (RN), Physician Assistant (PA) and Licensed Clinical Social Worker (LCSW) virtually
Across all settings of care, create a comprehensive and coordinated Plan of Care which takes into account information from medical records, the Patient and their families, Primary Care Physicians (PCPs), and Specialists as necessary
Ensure timely completion of all documentation
Coordinate discharge plans with the Extensivist Team on the day of discharge
In conjunction with the Practice Administrator, develop and communicate recommendations to C-Suite Leadership in an effort to continuously enhance care
Will collaborate for Specialty referrals, as appropriate
Communicate Plan of Care with Patients or their families/caregivers and the PCP during the longitudinal continuum of care and any changes in the Patient's health condition
Lead and participate in weekly Complex Case Management multidisciplinary meeting
Participates in On-Call Program which may include after-hours, weekends, holiday calls, and visitations

Supervisory Responsibility:
Clinical responsibility for the Licensed Clinical Social Worker (LCSW), Advanced Practice Registered Nurse (APRN), Physician Assistant (PA), Psychiatric APRN, and Registered Nurse (RN)
Extensivist Physician to report to Chief Medical Officer (CMO)

Work Environment:
The Extensivist Physician would operate remotely with assigned televisits with Patients as needed

Physical Demands:
This is a full time position and core hours of work and days are Monday to Friday 8:00 a.m. - 5:00 p.m.
Extended hours to accommodate Patient urgent and emergent care needs will be expected

Quarterly visits to Corporate office in Florida may be required

Required Education/Experience:
Minimum Experience: 5-7 years of Hospitalist, or Internist experience preferred
Medical Doctorate (M.D.) or Doctor of Osteopathic Medicine (D.O.); Internal Medicine
Medical License in the state of Florida
DEA license
Board certified in Internal Medicine (preferred)
Up to date CPR certification (BLS, ACLS, and PALS)

Additional Eligibility and Qualifications:
Ability to use Electronic Medical Record (EMR) system to document Patient encounters
Knowledge of ICD-10 Coding and STAR Measures attainment
Effective communication and interpersonal skills
Demonstrated ability to problem solve complex and multifaceted situations
Strong organizational, task prioritization skills
Computer literacy on G-Suite products and database programs
Satisfactory completion of a pre-placement Tuberculosis test is a requirement for this position
