-Starting salary: $53,000/ Year -(negotiable based on
experience)------------------------------------------------------------------------------------------------POSITION
OBJECTIVE: - The Specialized Field Case Manager (SFCM) will mentor
and support current case management staff to ensure the appropriate
assessment, planning, linkage, coordination, monitoring, evaluation
and advocacy to the child, family, foster family and primary
caregiver. - KEY FUNCTIONS: -
- Mentor and assist staff in identifying strengths, limitations,
risk factors, and needs, and to refer and coordinate the provision
of concrete services to the child, primary caregiver, and
biological families of children.
- Mentor and assist staff in providing written assessment of each
family, to include Ongoing Family Functioning Assessments, Case
Plans, Judicial Reviews, Safety Plans, and Family Support Plans.
Assist with monitoring and reviewing case plans with team
members.
- Mentor and assist staff with maintaining face-to-face and
telephone contact with each of the assigned eligible
children/families in the foster home, primary residence of the
child and/or family, school, day care, preschool, or any
appropriate setting.
- Mentor staff in arranging visitation schedules structured upon
the completed visitation plan (consistent with court orders and
safety plans) with input from the appropriate parties, supervised
visitations, and documenting observations as appropriate.
- Mentor and assist staff in notifying the biological parents,
GAL and court, of any changes in status, placement, medical &
clinical treatment, etc., relative to the child.
- When requested, assist the primary caregiver(s) and/or foster
family in implementing tasks and activities according to the case
plan.
- Assist staff in providing on-going support to the primary
caregiver(s) and/or foster family to assist the family in providing
a safe and secure home environment for the child.
- Intervene and assist Agency/CBC staff in crisis
situations.
- Mentor and assist staff in the provision of appropriate
information when applicable and accompany children to appointments,
as necessary, to report progress or lack thereof.
- Mentor and assist staff in developing and maintaining
appropriate documentation and concurrent planning in accordance
with Agency and/or CBC policy and procedures.
- May be requested to attend and/or participate in staffing
reviews, administrative reviews/staffing, judicial reviews,
permanency staffing, school staffing, CPT staffing, meetings, etc.,
as related to any casework completed by the SFCM. -QUALIFICATIONS:
REQUIRED:
- Bachelor's degree in social work or related area of study
- Related degrees include: - human services, criminology,
juvenile justice, psychology, sociology, counseling, special
education, education, human development, child development, family
development, marriage and family therapy, and nursing
- Minimum 1 year of experience as a Dependency Case Manager or
equivalent
- State of Florida Child Welfare Certification (CWCM) -PREFERRED:
- Master's degree in social work or related area of study (see
above)SKILLS: REQUIRED:
- Must be proficient in Microsoft Office Suite
- Conducts self in a professional manner with colleagues, peers,
and clients. Demonstrates professional behavior toward clients
consistent to the enhancement of the client's dignity and
well-being. Upholds established policies and professional standards
of conduct.
- Excellent verbal and written skills
- Strong communications & customer service skills
- Must be capable of completing assignments with minimum guidance
and supervision.
- Ability to objectively coach & mentor staff through complex &
difficult issues
- Demonstrated ability to identify, prioritize, and execute tasks
to meet deadlines.
- Ability to understand legal documents
- Ability to maintain a high level of confidentiality
- Ability to represent and promote positive and solution-oriented
behaviors within their team and the organization.
- Demonstrate cultural and linguistic competence & sensitivity to
population served
- Ability to travelSPECIAL CONDITIONS: -
- Thorough Background Screening upon hire including FBI, FDLE,
Local Law check, Employment Reference check, DMV check, Criminal
Records check, Social Security Trace & Drug Screening. - FDLE check
will be run at least every 5 years.
- On-call assignments- Yes, as assigned
- Travel- local: 50% per week, long-distance: 5% per
