Job Description:

SUMMARYResponsible for providing HIV Medical Case Management services to an assigned caseload in accordance with Federal legislation, the Ryan White Treatment Modernization Act of 2009, and the Orlando EMA HIV Health Services Planning Council. These services help consumers gain access to needed medical, social, educational, and other services. This includes direct assistance in gaining access to services, coordination of care, and linkage to appropriate services. Medical Case Management includes: (1) Initial assessment of service needs; (2) Development of individualized care plan; (3) continuous client monitoring and follow-up; and (5) Treatment adherence.ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.1. Initial assessment of service needs2. Development of a comprehensive, individualized care plan3. Timely and coordinated access to medically appropriate levels of health and support services and continuity of care4. Continuous client monitoring to assess the efficacy of the care plan5. Re-evaluation of the care plan at least every 6 months with adaptations as necessary6. Ongoing assessment of the client's and other key family members' needs and personal support systems7. Treatment adherence counseling to ensure readiness for and adherence to complex HIV treatments8. Client-specific advocacy and/or review of utilization of services9. Make appropriate referrals to Hope & Help's services and other service agencies.10. Perform oversight of the entire case and linkage to appropriate services.11. Maintain accurate, same day records of each client interaction via progress notes and corresponding documentation in the client file and meet all other reporting deadlines specific to grantee guidelines.12. Coordinate with Referral for Healthcare Specialists for all clients on shared caseloads.OTHER RESPONSIBILITIESPerforms other work-related duties and special projects as assigned.SUPERVISORY RESPONSIBILITIESThis job has no supervisory responsibilities.QUALIFICATIONSTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.KNOWLEDGE AND SKILL OF:--- Modern office practices, procedures, and equipment--- HIPAA Laws--- Concepts and principles of infectious disease in adult HIV-positive patients--- Counseling techniques and practices--- Principles and practices of social work--- Methods of compiling, organizing and analyzing dataABILITY TO:--- Communicate effectively both orally and in writing with a diverse population varying in age, physical/mental health, lifestyles, and cultural/ethnic backgrounds--- Meet time schedules, deadlines, and plan and organize work--- Work confidently with discretion and independently with little direction--- Work cooperatively with others using tact, patience, and courtesy--- Maintain current knowledge of technological advances in the field--- Work with confidential data with discretion--- Be sensitive to issues involved in working in a multi-cultural, community-based organization--- Interact effectively with patients and families from diverse populations--- Establish and maintain effective team relationships with other healthcare personnel and the public--- Respond effectively to client crisis situations; interview and assess needs of clients--- Follow through on all phases of the services for care--- Provide support to the Case Management Supervisor and other Case Management professionals to ensure the clinical team functions as a cohesive unit--- Possess and maintain valid Florida driver's license and proof of insurance--- Maintain reliable and accessible transportation for travel throughout Central Florida area--- Speak Spanish or Creole is preferredEDUCATION and/or EXPERIENCEBachelor's degree in a social science from an accredited college or university with at least one (1) year of case management experience, or Bachelor's degree not in a social science from an accredited college or university and must have at least one (1) year in direct HIV/AIDS case management experience.CERTIFICATES, LICENSES, and/or REGISTRATIONSValid Florida Driver's License required.PHYSICAL DEMANDSThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
